Interim Continuing Care Lead Nurse for Learning Disabilities required for a London based CCG for a 3 month contract to begin ASAP

My client, a Greater London based Clinical Commissioning Group are seeking to appoint a Lead Nurse for Learning Disabilities to help bridge the gap whilst they recruit to the position permanently.

To be considered for this position you must have the following skills and experience:

- Registered LD Nurse

- Experience of carrying out assessment of needs using decision supporting tools and making recommendations to CCG panels

- Case manage complex health care packages for CHC learning disability clients

- Experience of liaising with multi-disciplinary teams

- Working to the guidance of the NHS framework
